How to use this template

Use this template before Tower Block Radio workshops, events, live broadcasts, archive days, studio sessions or volunteer activity. The aim is to identify hazards, reduce risk and record who is responsible for actions.

Risk assessment table

For each hazard identified, record the following:

  • Hazard · What is the hazard?
  • Who might be harmed? · Who could be affected?
  • How could they be harmed? · What could go wrong?
  • Existing controls · What is already in place?
  • Risk rating before controls: Low / Medium / High
  • Further action needed · What else needs to be done?
  • Person responsible · Who will do it?
  • Deadline · When by?
  • Risk rating after controls: Low / Medium / High

Safeguarding check

  • Are children or adults at risk attending?
  • Is consent required?
  • Are emergency contacts needed?
  • Are staff or volunteers suitably briefed?
  • Is there a safeguarding lead for the activity?
  • Are DBS checks required for any role?
  • Is one to one contact avoided or managed?
  • Is online communication appropriate?

Accessibility check

  • Is the venue accessible?
  • Are toilets accessible?
  • Are instructions clear?
  • Is there a quiet space?
  • Is support available for forms or communication?
  • Are captions, transcripts or alternative formats needed?
